Doggie Daycare Delight
without specific permission:
1.
Doggie Daycare Delight, Inc. agrees to exercise due diligence and reasonable care, and to keep the premises sanitary and properly enclosed. All pets are handled or cared for by Doggie Daycare Delight, Inc. staff without liability on Doggie Daycare Delight, Inc’s part for loss or damage from disease, theft, fire, death, escape, injury or harm to persons, other pet(s) or property by said pet, or from other unavoidable causes, due diligence and care having been exercised.

Should any pet become ill or seem to be in need of medical consideration,Doggie Daycare Delight, Inc. reserves the right to administer aid and/or transport to, and be treated by, any available veterinarian. Any expenses incurred shall be paid by the Owner of said pet in addition to other fees incurred for services provided at or by Doggie Daycare Delight, Inc.
3.
Owner agrees to pay the rate for services in effect on the date their pet is checked into Doggie Daycare Delight, Inc. Prices are subject to change at any time, without notice. No pet will be released until all charges are paid in full. Cancellation fees may be applied for "
no shows
" or reservations cancelled less than 2 weeks in advance during peak season. Owner shall remain liable for all charges incurred for the care and maintenance of the pet listed on this contract. Unless otherwise arranged for in advance, multi-day frequency packages unused within the established timeframe will result in bonus days being forfeited or a prorated cost differential to be charged.

The Owner further agrees to be solely responsible for any and all acts or behavior of said pet while in the care of Doggie Daycare Delight, Inc.. The Owner of the pet agrees to pay reasonable attorney fees incurred by Doggie Daycare Delight, Inc. in the collections of any charges for services incurred by the pet’s Owner.
If your dog causes injury to another dog, either in daycare or boarding, and said dog needs to be treated by a veterinarian, you will be responsible for any and all charges.
5.
Pet must be in good general health and Owner must provide valid proof of all required vaccinations (Rabies, Distemper, Parvo, and Bordatella) on pet’s first visit to Doggie Daycare Delight, Inc., when vaccinations are updated, and/or annually. If your dog needs to get the bordetella vaccine, please get this vaccine TEN DAYS before your drop off date. Pet must also be on a scheduled prevention program during flea and tick season, or when specifically requested by Doggie Daycare Delight, Inc. Pets arriving with fleas and/or ticks may be bathed at Doggie Daycare Delight, Inc's discretion, at the Owner’s expense. Doggie Daycare Delight, Inc. reserves the right to refuse services or admittance to any dog.
6.
Pet must be picked up by Owner or a pre-approved Owner’s agent by 6:30 p.m. Charges may be incurred, at prevailing rate, for late pick-up. Pet will only be released to an Owner or pre-approved Owner’s agent, as listed above. If Owner’s agent is not specifically listed above, Owner must provide either written or verbal authorization directly to Doggie Daycare Delight, Inc. granting permission for the pet to be picked up by another individual; such permission will be considered specific to that day until or unless this form is revised by Owner.
7.
All attending pets will have up-to-date/readable identification on their collars bearing, at a minimum, their first name, and will be safely escorted into and out of Doggie Daycare Delight, Inc. on leash.
8.
Owner agrees that their pet may be videotaped, photographed, and/or recorded. Doggie Daycare Delight, Inc. shall be the exclusive owner to the results and all proceeds of such tapings, photography, and recordings with the rights throughout the world, an unlimited number of times in perpetuity, to copyright, to use and to license to others in any manner. Owner further agrees that their pet may be used in any and all media and in the promotion, advertising, sale, publicizing, and exploitation of Doggie Daycare Delight, Inc.
9.
I understand that under no circumstances will Doggie Daycare Delight, Inc. be liable for consequential damages or damages beyond the replacement value of my dog not to exceed $200.
10.
I understand that Doggie Daycare Delight, Inc. is a cage-free facility. I accept the risks involved and agree that Doggie Daycare Delight, Inc. is not liable for any injuries or illnesses resulting during my dog's attendance.
11.
I hereby declare to Doggie Daycare Delight, Inc. that I am the legal owner of my dog; that my dog has not been exposed to Distemper. Rabies, or Parvo within the past (30) thirty days; that my dog has been inoculated as indicated by records presented; that my dog is currently and properly licensed; and that I (the owner) have read this agreement in it's entirety.
12.
Although Doggie Daycare Delight, Inc. screens the dogs for temperament, watch the dogs carefully. and do not take aggressive dogs, day care can be hazardous due to dogs playmg together. They can get rambunctious at times and we cannot be held responsible for injuries and /or death that may occur in and out of the day care including the transporting of animals.
